Driver Evaluations (Doctor Ordered)

This is a 90 Minute Adult Driver Evaluation at a cost of $175.00 

You will drive a PDS Car with a trained Driving Instructor

The Driving Instructor will make an observational record on how you manage during various driving situations.  This record will be sent to the referring medical professional. We do not determine if you can legally continue to drive. 

Adult Being Evaluated: 

• Complete the Patient Driver Evaluation Registration form below.

Medical Professional: A Prescription from a Medical Professional is Required for an Evaluation.
